This survey is for supervisors of students participating in the Arts Practicum.

It is intended as a 'check-up' in the early stages of placement, and must be submitted when the practicum student has completed around 30 hours work on their project. It may raise issues that you would like to discuss further with your student in order to make the practicum run more smoothly. If neccessay, the Arts Practicum Co-ordinator can contact you to follow up any problems identified in the survey.
